Any medical records from your previous veterinarian

A list of medications your pet is currently taking

Any concerns or symptoms you’ve noticed

A fresh stool sample (for puppies, kittens, or new rescues)

You may also Fill out our New Client Form ahead of time to speed up the check-in process. Just ask us to send it via email or download it from our website (if applicable).
